The award-winning Leonard Florence Center for Living Green House, is a revolutionary, state-of-the-art skilled nursing residence. The Center features a home-like environment with every modern convenience. Since it's opening in 2010, the Center has been recognized throughout the world as an innovative model of care for seniors. And now, in 2020, we have been chosen to implement a new vent unit expansion! The unit will be equipped with State of the Art Technology from the Vent Unit, to the resident having the ability to operate all aspects of their environment remotely. The candidate will be provided a structured education plan to assist in their growth and development on Vent Care. Continuing Education will be ongoing and at the forefront of this Vent House Expansion. We will be accepting eager CNA's, willing to learn and have the desire for professional growth.  

Certified Nursing Assistant (SHAHBAZ)

Full time, Part time and per diem positions available on ALL SHIFTS!

8 hour and 12 hour shifts are available.


Life in the Green House is organized around the needs and lives of the residents who live there.  The constant companion of the resident is the C.N.A. (Shahbaz). The Shahbaz is a certified nursing assistant (CNA) with additional specialized training. It is the duty of the C.N.A. to protect, to nurture and to sustain the resident.  C.N.A.'s  are responsible for maintaining the safety and cleanliness of the environment and for providing high quality of care.  The work the C.N.A.  does is organized around a self-managed work team.  Members of the self-managed work team serve as coordinators to facilitate the delivery of and compliance with all services and tasks that they are responsible for under the care plan and operationally.  C.N.A. 's are coached by the Green House Guide, who oversees operations and assures compliance with all regulatory and organizational requirements.  The C.N.A. s  work cooperatively with the Clinical Support Team (CST/Nurse) to support their work.

  • Recognize and respond to the needs of the residents.
  • Promote autonomy of residents in decision-making.
  • Maintain confidentiality and privacy of resident care, procedures and documentations.
  • Communicate with co-workers at all levels to meet the needs of the residents.
  • Meet with the CST/Nurse as scheduled or needed to discuss residents' needs and progress in identifying and correcting problems.
  • Observe, monitor and report to the CST any changes in condition.
  • Document observed data per organization policy.
  • Attend monthly house meeting.
  • Take your breaks away from houses when possible.
  • Eat meals with the residents at the dining room table.
  • Weights at the time of admission.
  • Assist residents with personal care functions, including:
  • Dental and mouth care;
  • Bathing;
  • Dressing and grooming;
  • Hair care;
  • Shaving;
  • Bowel and bladder care;
  • Other personal and hygiene care;
  • Assist with and/or perform restorative and rehabilitative procedures as outlined and assigned by the care plan and with appropriate training.
  • Measure and record vital signs such as temperature, pulse, respirations, weight, height and anatomical dimension/circumference.
  • Assist moving, positioning and transporting residents into and out of beds, chairs, showers, wheelchairs, lifts, etc.
  • Maintain a manual-lift free environment by utilizing the lift systems provided in the houses.
  • Assist with the application of slings, elastic bandages, binders, etc.
  • Provide passive range of motion exercises as outlined and assigned by the care plan and with appropriate training.
  • Prepare and serve meals, between meal, bedtime and other nutritional snacks per the posted menu and the stated preferences of the resident.
  • Observe standard precautions per organizational policy.
  • Keep all areas of the home clean, performing routine housekeeping duties.
  • Make occupied and unoccupied beds and change bed linens.
  • Launder resident's clothing and other personal articles.
  • Follow established safety precautions and observe, monitor and intervene in or report to the Guide any unsafe conditions.
  • Assure resident calls for assistance are answered promptly.
  • Report accidents and incidents when they occur.
  • Report and abuse or suspected abuse to the Guide immediately.
  • Follow established smoking regulations and report violations.
  • Attend and participate in orientation programs, on-going training and educational classes.
  • Perform other related duties as required by the residents.
  • Fill all O2 tanks at the end of each shift.
  • Clean kitchen before end of shift.
  • Meet each new admission at his/her room. Orient to room and make comfortable.
  • In morning, open curtains and pull up shades.
  • Keep living room tidy throughout your shift.
  • No sleeping ever.
  • Keep back of the house stocked and tidy. 11-7 to complete all laundry by end of your shift.
  • Listen to report at the beginning of your shift, complete all paper work at the end of each shift. Look at bulletin board in kitchen for notices. When you have down time, spend it with the residents in some type of activity.


  • Current State CNA Certificate
  • Graduate of an approved CNA Training Program
  • Must be able to read, write and speak the English language
  • Must possess the ability to make independent decisions when circumstances warrant such action
  • Must possess the ability to follow a nursing care plan
  • Must have patience, tact, a cheerful disposition and enthusiasm as well as being able to handle residents on whatever maturity level in which they are currently functioning
  • Must be able to cope with the mental, emotional and physical stress of the position
  • Must be able to work with the ill, disabled, elderly, emotionally upset and, at times, hostile people within the facility
  • Ability to stand, sit, walk, bend and squat for prolonged periods
  • Ability to push, pull objects and lift/carry up to 20 pounds unassisted on a frequent basis and up to 50 pounds or more with assistance on occasion
  • Ability to interact effectively with residents, family members, visitors, government agencies/personnel, the general public and other staff members
